As an Admissions Nurse, you will experience a rewarding work life in a full time, salaried position.
- 3- 12 hour shifts Thursday, Friday and Saturday 9a-9p.
Responsibilities:
- The Admissions Nurse is the first point of contact for patients, families, and caregivers beginning their hospice journey.
- Conducts the initial assessment to determine hospice eligibility, review the patient’s medical history, and gather information regarding the patient’s personal history and goals to determine how the hospice team can best meet their needs.
- Also, assess physical, psychological, social, and spiritual needs of the patient/family/primary caregiver and initiates appropriate referrals, interventions, and discipline visit frequencies.
- Completes skilled nursing care as needed during admission visit per direction of physician’s order and in line with patient’s goals of care.
- Coordinates with Patient Access Specialist to conduct scheduled and urgent informational/sign-on and admission assessments for Emmanuel Hospice referrals and ensures paperwork and documentation is completed accurately and timely.
- Educates patients, family, and caregivers about disease, signs and symptoms of dying, symptom management, and covered/not covered services and medications.
- Participates in non-admission follow-up plan of care and communicates with referral sources to provide appropriate alternatives for patients who do not meet hospice criteria.
- Creates accounts, evaluates, and orders initial medical supplies, durable medical equipment, and medications to meet patient needs.
- Actively participates in Admissions Team meetings by reporting on recent referrals, sign-ons, and admissions.
- Actively engages and communicates with the patient’s Primary Care Physician as well as the Hospice Physician to determine eligibility and reviews appropriate relatedness of services and covered medications.
Qualifications:
- Current RN license in the State of Michigan is required and ability to receive credentialing at partner hospital(s)
- BSN or master’s degree preferred. Associate degree/diploma required.
- 2 years of nursing experience required, 1 year experience in acute care unit, hospice, and/or admissions experience preferred.
