A 28-Day Rotation Designed for Flexibility & Fulfillment
- Bi-weekly schedule: 7 days on, 11 days off — then 3 days on, 4 days off

About the Role: Extended Care RN
In this rewarding after-hours, on-call role, you will:
- Triage and manage after-hours patient needs via phone and onsite visits
- Conduct hospice informational visits and nursing admission assessments
- Support patients and families by collaborating on individualized Plans of Care
- Work alongside an interdisciplinary team to educate, advocate, and provide holistic, compassionate care
- Deliver exceptional service while making a real impact during the end-of-life journey
