What are the responsibilities and job description for the Hospice Admissions Nurse position at Emmanuel Hospice?

As an Admissions RN at Emmanuel Hospice, you will serve alongside a dynamic team in a full time, salaried position.
Responsibilities
- Working a rotating schedule of every other Saturday and Sunday 9am - 9pm and a combination of 4 weekdays per pay period from 9am – 9pm (72 hours per pay period) along with additional meetings and holidays as required for the role.
- Conducting informational and sign-on visits
- Completing comprehensive RN admission assessments
- Serving alongside our extended care team to provide nursing services to the dying and their loved ones in a 50 mile radius of Grand Rapids, MI.
- Collaborating with referral sources, patients, families, and healthcare professionals to establish an initial plan of care
- Communicating effectively with other hospice registered nurses and other staff to ensure coordinated care is provided
- Assists with follow-up contacts and re-assessments as needed
Qualifications
- Ability to relate with and educate patients and families on the hospice benefit
- Willingness to be a vocal advocate for Emmanuel Hospice with referral sources
- Excellent time management and organizational skills with emphasis on attention to detail when completing paperwork
