Referral/Admissions Manager

Building our loving service in Colorado for nearly 100 years, Dominican Home Health Agency now has a new name and focus: Emmaus Catholic Hospice, a non-profit agency.
JOB SUMMARY: Serves as a facilitator/coordinator for all referral, admission, intake processes for the organization assuring adherence to agency policies and federal and state regulations; serves as liaison between clinical staff and operations staff; and as liaison among the Agency, facilities, the medical and general community.
Duties and Responsibilities:
- Assumes responsibility for facilitation and coordination of all staff and services involved in the referral, admission and intake processes (including, but not limited to) as follows:
A. Referral Process:
1. Intake call completion
2. Development of referral chart, transition from pending to accept
3. Request of medical records/follow through with completion/download to charts
4. Coordinate informational visit if requested- could also do visit
5. Coordinate contract process in collaboration with Director, Clinical Services
6. Manage ongoing correspondence related communication notes
7. Obtain VCTI from Hospice physician- document in chart
8. Coordinate Consent/admission dates/process- appropriate consent for Transfers
B. Admission/Transfer Process
1. Coordinate date/time/team
2. Verification of Medicare eligibility
3. Validation of appropriate consents
4. Finalize chart detail after admission- Care location, admit settings, etc.
5. Submit HPS coding detail
C. Other Intake processes (additional)
1. Assist in Management of referral tracking system/process (i.e. Wellsky CarePort)
2. Liaison with State Serve/Guardian for admission/transfer process
3. Audit Intake process (in home visits and documentation)
- Always maintains availability during operating hours and participates in all activities relevant to the professional services provided.
- Assures all clinical functions/services meet all state and federal regulations and rules.
- Responsible for the facilitation, coordination, and supervision of services and takes steps to ensure: (1) The quality of services is maintained, (2) Staffing of the services provided is appropriate, (3) Services are available in accordance with all contracts and regulations.
- Assures all services are provided by a qualified team member.
- Develops and/or approves and participates in on-going education.
- Demonstrates a positive attitude as example of professional behavior for both the office staff and direct care staff.
- Provides, as needed, direct coordination of intake processes with clinical staff and assures processes are completed per regulations and agency policy including documentation of same.
- Participate in the field according to staff needs, providing spot checks and follow‑up for specific services and/or quality improvement issues.
- Provides direct response and guidance to staff for problem resolution in patient care as it related to intake processes.
- Serves as a liaison between vendors and clinical staff as it relates to intake processes.
- Assists with management of IT referral system, tracking of referral data.
- Assists Director, Clinical Services and Social Worker with coordinating Respite Care for patients when needed.
- Always represents the Agency in a professional and loyal manner. Always demonstrates professional deportment and communication style with the following, but not limited to, all office staff, field staff, contractors, vendors, and patients and physicians and their offices.
- Initiates joint projects and presentations to improve patient care and/or education of community members on issues pertinent to agency interests.
- Participates in Quality Assessment Performance Improvement (QAPI) activities as assigned by Administrator/DCS.
- Assists with AOC duties as needed and assigned by Administrator/DCS.
- Other duties as assigned.
Qualifications/Education Required:
- Must be a practicing Catholic in good standing.
- Registered Nurse with current state licensure without stipulations, and
- Bachelor’s degree required, Master’s degree preferred.
- At least two (2) years clinical nursing experience; two (2) years in hospice preferred, closely related nursing accepted.
- Experience in supervision of at least one (1) year.
- Must have proven supervisory and communication skills.
- Must have proven ability to act as liaison among clinical and non-clinical employees and the public.
- Should have budget development and monitoring abilities.
ABILITIES, SKILLS, AND KNOWLEDGE:
- Demonstrated understanding of the Medicare and Medicaid reimbursement system.
- Demonstrated expertise in coordinating work activities of inter-disciplinary team.
- Excellent oral and written communications skills with proven ability to communicate with technical and non-technical users.
- Ability to read, analyze, and interpret common scientific and technical journals, professional journals, and legal documents.
- Ability to read and interpret physicians’ orders and notes from other caregivers as well as respond to them verbally and in writing.
- Ability to effectively present information to clients and employees and community members.
- Ability to respond effectively to the most sensitive inquiries or complaints.
- Ability to define problems, collect data, establish facts, and draw valid conclusions.
- Ability to send/receive emails and use the internet for research and informational purposes.
- Ability to use software to create a spreadsheet with minimal instruction.
- Ability to use word processing software for communication and documentation purposes.
- Ability to use independent judgment to manage and impart confidential information.
- Ability to prioritize multiple time-sensitive tasks simultaneously.
- Working knowledge of Windows operating system and ability to advance computer skills as required.
- Ability to establish and maintain effective working relationships with fellow employees and the public.
SPECIAL REQUIREMENTS:
- Current Basic Life Support (BLS) certification.
- Pre-employment drug and TB screening. Annual TB screening thereafter.
- Proof of current flu vaccination or approved Medical or Religious exemption.
- Possession of a valid state Driver's License with reliable transportation and applicable car insurance.
PHYSICAL REQUIREMENTS:
- Ability to regularly sit, stand, stoop, bend, kneel, walk, balance, crouch, crawl, push, pull, reach with hands and arms, and talk and hear.
- Ability to frequently use hands to finger, handle, or feel.
- Ability to frequently lift and/or move up to 50 pounds and occasionally lift and/or move 100 pounds with assistance.
- Ability to use close vision, distance vision, color vision, peripheral vision, depth perception.
- Ability to adjust focus.
WORK ENVIRONMENT:
- The employee is frequently exposed to outside weather conditions.
- The work environment is highly variable, as care is provided in residential spaces.
- Lighting, acoustics, air quality, temperature and environmental hazards are not predictable.
- The employee is occasionally exposed to wet and/or humid conditions, household pets/animals, fumes, or airborne particles, and toxic or caustic chemicals.
- The noise level in the office work environment is usually moderate.
TRAVEL:
The employee is required to travel frequently to complete agency business.
COMPENSATION:
- Benefits for full time employees (>32 hours/week): Health Insurance (Aetna): Dental Insurance (Delta Dental); Vision (VSP). Health, Dental, and Vision insurance premiums for full time employees are paid at 100% by the agency for employee and at 75% by the agency for spouses and dependents. Additional benefits include: FSA, Short and Long-term Disability, Life Insurance; 403b investment/retirement plan; Accrued PTO and 8 Paid Holidays.
- Salary $56 - $58 hourly, commensurate with experience.
