Job Summary:
As a Client Care Coordinator at EmmUcare Home Health, you will be the key liaison between our clients, their families, and our caregiving team. You will coordinate and oversee care services, ensuring that each client’s needs are met efficiently and effectively. The ideal candidate will have excellent communication skills, strong attention to detail, and a passion for providing exceptional care.
Key Responsibilities:
- Coordinate and oversee client care plans, ensuring that services are delivered as per the client’s specific needs and goals.
- Serve as the primary point of contact for clients, families, and caregivers, addressing concerns and providing updates as needed.
- Monitor the quality of care provided and make adjustments to care plans as necessary to meet client needs.
- Ensure compliance with all regulatory and agency standards, including HIPAA and patient confidentiality.
- Conduct client assessments and regular follow-ups to ensure satisfaction and improve care delivery.
- Collaborate with the clinical team, management, and other staff to address client needs and resolve any issues that arise.
- Maintain accurate and up-to-date records of client care and communication.
- Provide support and training to new or existing staff as needed.
Qualifications:
- Previous experience in a healthcare or home health setting preferred.
- Strong organizational and multitasking skills.
- Excellent verbal and written communication skills.
- Compassionate and patient-centered approach to care.
- Ability to work independently and as part of a team.
- Proficiency in Microsoft Office and basic office software; experience with care management software is a plus.
- Knowledge of home health industry standards, regulations, and best practices.
