We are seeking a capable Care Management Manager who has acquired broad experience in caring for patients, and has developed a sound understanding about the care of a particular patient population.The Care Management (CM) Manager is responsible for direct oversight of the daily operations of the Care Management department as it relates to Discharge Planning.
Oversees departmental compliance by ensuring that the team adheres to established policies and procedures.
Monitors quality and productivity standards in order to manage individual performance as well as the teams overall performance.
Responsible for coaching, developing, training, and the performance management process for employees.
Ensures that department, division, and system strategic goals are connected to the daily work of the department.
Understands metrics such as LOS, avoidable days, readmissions, saved days and will be able to implement measures in order to achieve targeted goals.
Through ability to effectively communicate and organize workflow, must quickly and effectively break down barriers to discharge.
Displays a broad understanding of complex discharge planning and strength in problem solving and critical thinking.
MINIMUM QUALIFICATIONS :
If a RN, Bachelor's degree required. Master's degree preferred.
Must have a valid, active unencumbered Nursing license or temporary permit approved by the Georgia Licensing Board.
LCSW will be accepted in lieu of RN requirements.
ACM or CCM certification preferred within a year of hire or eligibility.
3 years of experience in Case Management (CM) required.
3 years healthcare experience required.
Leadership experience preferred.
Proven ability to coach, mentor, and lead.
Demonstrates leadership abilities.
At least two years of experience in a healthcare setting is required.
Two (2) years of insurance verification, authorization, or related work preferred.
