EPIC, a member of Empath Health is currently seeking a Housing Case Manager to join our team.
Full Time; hours may vary; travel required throughout Hillsborough county area.
The Housing Case Manager is responsible for coordination and case management for a diverse population of individuals/clients who are experiencing homelessness with the goal to acquire and maintain housing stability. Depending on the specific program, the individuals/clients served may include persons infected with HIV and/or youth and young adults ages 16-24 who are experiencing homelessness & may qualify for housing through the Youth Homelessness Demonstration Program.
Job Duties/Responsibilities:
- Supports and embodies the Empath Health mission, vision and values.
- Assess the financial needs of the clients and implements appropriate assistance.
- Screens individual referrals from case management agencies to assess the eligibility criteria for needs based assistance.
- Establishes working relationship with utility companies
- Makes payments to vendors after determining eligibility
- Ability to network with community organizations and agencies
- Ability to deal professionally, courteously, and efficiently with clients, service providers and other employees
- Develop individualized stability plans with assigned clients
- Provide permanent housing placement services in accordance with contract and / or funding requirements
- Identify problem areas, suggest and implement solutions and monitor progress.
- Ensure and safeguard the human and legal rights of clients, their families and co-employees at all times
- Ensure accurate and timely completion of all required paperwork or documentation, including but not limited to service activity logs, case notes, intakes, assessments and required forms.
- Act as an advocate within the company and community regarding the needs of the clients and their families
- Develop and maintain positive working relationships with other area agencies that offer services needed or used by clients.
- Attend and participate in staff and team meetings
- Keep accurate records of all those activities identified above and others as assigned
Qualifications/Requirements:
- Education and/or Experience: Bachelor’s degree (B.A.) from four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience.
- Substantial successful work experience in the field of office management may be substituted for the degree.
- Bilingual preferred.
- Ability to write and interpret business documents, including use of proper grammar, spelling, and punctuation.
- Ability to work a flexible work schedule as needed.
- Knowledge and ability to apply general office practices and procedures.
- Knowledge and ability to apply general math pertaining to percentages, allocations and discounts.
- Computer skills using current software, including but not limited to Microsoft Office products
- Knowledge of local community resources.
- Must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion, including making home visits/inspections. Must provide proof of valid automobile insurance, a copy of which will be placed in the employee’s HR file.
- Housing Case Managers hired to work as part of the Youth Homelessness Demonstration Program are required to travel on an ongoing basis throughout Hillsborough County using their own vehicle.
