What are the responsibilities and job description for the Patient Concierge position at Empath Health?
Suncoast Hospice, a member of Empath Health is currently seeking a Patient Concierge to join our team.
Schedule: M-F 9a-6p
The Patient Concierge:
- Is the first responder from our agency who is passionate about the services we offer and works directly with patient/family/authorized representative, or any other customer to insure optimal access.
- Keeps patient centered services and caring at the forefront, exceeding customer expectations and insuring best practice and flawless care in all aspects.
Position Requirements:
- Education and/or Experience: High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.
- Excellent verbal and written communication skills.
- Knowledge of medical terminology.
- Computer skills.
- Knowledge of Empath Health/Suncoast Hospice programs.
- Basic knowledge of reimbursement for services.
- Working knowledge of community resources.
- Ability to listen, assess needs, and coordinate care.
- Possess appropriate license and vehicle for travel needed to perform the job responsibilities.
