What are the responsibilities and job description for the PRN LPN Case Manager position at Empath Health?
Tidewell Hospice, a beautiful Southwest Florida healthcare organization, is seeking an experienced and team-oriented LPN to join our PRN Team for Charlotte and Southern Sarasota county areas.
Under physician’s orders and RN supervision, the Licensed Practical Nurse (LPN) is responsible for the observation of patient/family needs and providing palliative and supportive care to the patient/family unit in the home setting environment, assisted living facility environment, and the long term care environment
Educational/Professional:
1. Graduate of approved school of practical/vocational nursing and current Florida LPN license.
2. One year of recent experience in acute health care experience required.
3. Previous Hospice/Home Health Care experience preferred.
4. CHPLN certification is strongly encouraged within 3 years of employment.
IV certification required within 6 months of hire.
