Empath Home Health, a member of Empath Health, is seeking a Quality Improvement Nurse.
The Quality Improvement Specialist is uniquely positioned and is responsible for monitoring internal and external quality measures across the agency. The Quality Improvement Specialist proactively participates in the development and implementation of the organization’s strategic initiatives, improves performance, and creates value for both internal and external customers. Responsible for managing HIS, customer complaints, performing chart audits, rounding, performance improvement projects and analyzing key performance indicators to ensure we identify areas of improvement and to meet regulatory and quality standards.
Requirements:
- Education and/or Experience: Registered Nurse or healthcare professional with minimum of Associates Degree in related field, Bachelor’s Degree preferred. (A combination of work experience and education may be considered as meeting the requirement)
- Understanding of Hospice, Home Health or Palliative Care standards and quality measures. (HIS and OASIS data set and data collection preferred)
- Experience monitoring healthcare external and internal quality measures guiding performance improvement projects.
- Strong critical thinking and problem-solving skills
- Must be able to work effectively with diverse audiences including executive leadership, physicians, nurses, aides, allied health professionals, and a wide variety of non-clinical staff.
- Exceptional communication, organizational, facilitation skills and a strong team principle
- Ability to provide guidance and expertise to project teams and continuous improvement oversight to team members by monitoring individual project plans.
- Superior organizational skills; able to prioritize and manage multiple tasks while meeting deadlines with minimal supervision.
- Knowledge of quality improvement methods and models (e.g. Rapid Cycle, Lean Six Sigma, Baldrige, PDSA)
- Proficient in Microsoft Office Applications including Excel, Word, and Power Point to create reports.
- Ability to learn internal software applications necessary to complete primary task and responsibilities.
- Experience in customer service. Strong customer advocate committed to the continual improvement of quality, safety, and service excellence.
- AHCA, CHAP and Joint Commission experience preferred.
