Summary of Responsibilities :
The Clinician provides clinical and wellness services to CHA residents within the Quality-of-Life Services model.
Primary Responsibilities :
The clinician provides clinical services to CHA residents, including assessments, treatment plans, individual and family services, referrals and advocacy. The clinician ensures ethical treatment of clinical issues and provides consultation and support to FamilyWorks employees with regard to clinical issues.
Working and Reporting Relationships :
Reports to the Senior Clinician and works in collaboration with E&ES staff. Will also maintain positive relationships with CHA staff and partner agencies.
Functions and Duties :
- Caseload will consist of clients that request or are referred for clinical and wellness services.
- Outreaches clients via various outreach methods (In-Person, Telephone, Email / Mail correspondence, social media, etc.) to engage in services.
- Conduct Quality of Life assessments with clients that request services. Identify clinical and wellness services that are needed and connect client with services.
- Develop treatment plans, provide individual and group counseling services, and advocacy when appropriate.
- Provide wide range of clinical services to individuals, groups and families including therapy, support and crisis management
- Develop appropriate referrals to community services / supports
- Train E&ES staff on how to identify signs of Substance Abuse / Abuse and Neglect / Domestic Violence / Mental Health issues
- Assist clients in application and management of SSI / SSDI benefits
- Provide guidance / consultation to staff when Crisis Intervention is needed
- Participate in Case Staffings
- Manage or assist to the management of cases with clinical issues
- Act as a support to Case Coordinators, Resident Service Coordinators and Career Coaches
- Utilize strong time management and organizational skills
- Maintain quality and timely documentation for services provided– assessments, case notes, etc.
- Other duties as assigned
