What are the responsibilities and job description for the CASE MANAGER position at Employment Solutions Inc?
LifePath is looking for a dynamic case manager to join our clinical team consisting of a care coordinator, multidisciplinary therapists, and a nurse practitioner. Individuals and families served by LifePath often have needs for assistance with housing, employment, education, medical treatment, transportation, and more. The case manager will be expected to assess each client’s needs, implement a plan for services, and monitor the supports facilitated to evaluate progress towards meeting client goals. The case manager will connect clients with the resources and supports needed to assist with achieving and maintaining function independence.
Job Duties:
- Provide reliable, person-centered service to a diverse population
- Support LifePath’s mission of quality behavioral health service, community collaboration, lifestyle wellness, and service integrity
- Collaborate and develop service plans reflective of person centered goals, and evaluate progress towards achieving goals
- Advocate for clients in a variety of settings to promote autonomy and self-directed decision making
- Collaborate with a multidisciplinary clinical team
- Effectively complete all required service documentation
- Offer proactive, creative solutions contributing to LifePath’s service delivery
- Adhere to professional standards related to case management service delivery
Qualifications:
- A minimum of 3 years of work experience in case management and/or relevant experience in a related field
- Ability to demonstrate relevant case management knowledge and skills
- Solution focused approach to problem solving
- Dynamic service-oriented work ethic
- Ability to communicate effectively and collaborate with interdisciplinary team members
- Bachelor’s degree in a human services related field
