As the MDS Nurse Coordinator, you will be responsible for the timely completion of accurate assessments and interdisciplinary care plans that meet Federal and State guidelines.

As the MDS Nurse Coordinator, you will be responsible for the timely completion of accurate assessments and interdisciplinary care plans that meet Federal and State guidelines.
Job Responsibilities
- Identify resident acuity and needs, help to determine specific care needs, and communicate needs/expectations to families and responsible parties
- Complete assessments, PDPM, and care plans for all residents assigned
- Monitor completion of PDPMs by other disciplines within time frames prescribed by regulatory guidelines
- Advise supervisor of incomplete and/or untimely assessments by disciplines other than nursing
- Ensure accurate, timely completion of the Triggers sheet for assigned residents
- Initiate care plans and supporting activities that will result in the best possible outcome for assigned residents
- Monitor and ensure completion of 7-day look-back documentation by nursing staff within prescribed time frame
- Maintain current knowledge of the PDPM assessment process
- Run the Restorative program in the community
- Responsible for Quality Measure Program
- Licensed RN (preferred) or LPN with the state of Illinois
- 2 years MDS and Care plan experience
- Knowledge of Medicare MDS and PDPM
- Experience in Restorative Programming, Fall care, monitoring, and investigation prevention
- Knowledge of Medicare and Medicaid reimbursement
