Job Summary
The Case Manager plays a crucial role in supporting individuals facing various challenges typically encountered by formerly incarcerated individuals. This position involves assessing client needs, developing personalized reentry plans, and coordinating services to promote overall well-being and reentry success. The ideal candidate will possess a strong background in social work, experience in the nonprofit sector and demonstrate a commitment to empowering clients through effective case management.
Responsibilities
- Conduct comprehensive assessments to identify client needs and strengths.
- Develop and implement individualized service plans that address client goals.
- Facilitate access to community resources, including public transportation, safe housing and second chance employers.
- Monitor client progress and adjust service plans as necessary.
- Provide crisis management support to clients in urgent situations.
- Collaborate with multidisciplinary teams to ensure holistic care for clients.
- Utilize motivational interviewing techniques to encourage client engagement and self-efficacy.
- Maintain accurate and up-to-date documentation of client interactions and progress.
- Monitor recidivism rates for EWOP graduates while staying current on state and national recidivism rates.
Skills
- Experience in social work with a focus on proven case management principles.
- Proficiency in crisis management strategies to effectively support clients in distress.
- Familiarity with Second Chance Employer opportunities and workforce development initiatives to assist clients in gaining employment.
- Knowledge of transitional living facilities and other community resources.
- Excellent communication skills, both verbal and written, for effective client interaction.
- Ability to build rapport with diverse populations while maintaining professional boundaries.
- Strong organizational skills to manage multiple cases efficiently.
