Overview :
We are seeking a compassionate and dedicated Adult Rehabilitative Mental Health Worker to join our team. In this role, you provide support, guidance, and therapeutic interventions to adults living with mental health conditions. The ARMHW collaborates with clients to promote recovery, independence, and overall well-being through individualized treatment plans, skills training, and community integration support.
Responsibilities:
- Client Support: Establish and maintain positive relationships with clients to create a supportive and safe environment
- Individualized Treatment Planning: Develop, implement, and review individualized rehabilitation plans in collaboration with clients and clinical supervisor as needed.
- Skill Development: Assist clients in developing daily living skills, social skills, and coping mechanisms essential for independent living.
- Crisis Intervention: Provide immediate support and intervention during crisis situations, ensuring client safety and well-being.
- Case Management: Coordinate services and referrals to community resources, including housing, employment, and healthcare services.
- Documentation: Maintain accurate and up-to-date records of clients interactions, progress, and treatment plans in compliance with legal and ethical standards.
- Team Collaboration: Work collaboratively with a multidisciplinary team including therapists, psychiatrists, and other healthcare providers to deliver comprehensive care.
- Educate clients about available resources. Advocate for clients' needs and rights within various systems.
- Community Integration: Support clients in participating in community activities of their choice and developing social connections as needed
Working Conditions:
- Flexibility to work in various settings, including clients' homes, community centers, Libraries and healthcare facilities
- May require evening and weekend availability based on client needs.
