JOB SUMMARY:
Under the general supervision of the BHS Director, provides clinical social work services for patients and their families and coordinates care with patients, families, and the health care team. Exhibits the F.I.R.S.T. values (Friendliness, Innovation, Respect, Service, and Trust).
ESSENTIAL FUNCTIONS, DUTIES AND RESPONSIBILITIES:
- Makes social assessments of patient's total needs, including documentation of the patient's social, emotional, financial and family situation.
- Develops plans and coordinates therapy for patients and families based on assessment.
- Cross trains to perform intake assessments on patients to assess for appropriateness of admission to the inpatient unit, and facilitates admission process for patient.
- Provides BHS screens, suicide risk assessments, and consults to the acute medical floor when requested.
- Provides group and individual counseling as well as crisis-counseling for patients, families, and visitors.
- Provides education and training to the staff regarding individual roles in social services and discharge planning as needed.
- Assures that performance improvement activities for social services are performed and that actions are taken to ensure delivery of quality care.
- Assures interpreting needs of patients and their families are met.
- Arranges for patient discharges to institutions, personal care homes, nursing homes as needed.
- Considers the age of the patients served, applies knowledge of growth and development and age-specific techniques through life span (neonates/infants, pediatrics, adolescents, adults, and geriatrics) in planning and implementing care.

- Provides in-services in the community when assigned.

MINIMUM QUALIFICATIONS OR EQUIVALENTS:
Education: Masters degree from an accredited school of social work.
Experience: Two years experience as social worker and/or discharge planner in psychiatric clinical setting preferred.
LICENSES, CERTIFICATIONS, AND/OR REGISTRATIONS:
Kentucky Licensed Clinical Social Worker. Non-Violent Crisis Intervention (CPI) within 3 months of hire and every 2 years to maintain certification.
Shift: 1st Shift
