We are seeking a full-time Hospice Social Worker who would like to join our hospice team!
Job Description :
- Hospice Social Worker, M-F, Dayshift
- Create your own visit schedule and have work / life balance..
- As a Hospice Social Worker, you will provide patients and families education and support throughout the end-of-life process and address patient's physical, emotional and spiritual needs.
- You will work closely with the hospice interdisciplinary team to create and implement a holistic plan of care congruent with the patient's and family's wishes, focusing on comfort, support and quality of life.
- Pay : Starting at $25 / hour, with guaranteed minimum of 40 hours paid per week, even if you work less.
