We are seeking a full-time hospice RN who would like to join our hospice team!
Job Description:
- M-F, Dayshift (8 am-5 pm, with opportunities to work 8 am-3 pm)
- As a hospice Case Manager, you will provide patients and families education and support throughout the end-of-life process and address patient's physical, emotional and spiritual needs.
- You will work closely with the hospice interdisciplinary team to create and implement a holistic plan of care congruent with the patient's and family's wishes, focusing on comfort, support and quality of life.
- Pay: DOE. Starting at $43/hour ($89,440 annually), with guaranteed minimum of 40 hours paid per week, even if you work less.
