Position Summary:
Physical Therapist provides therapy services to patients in a home health environment by assessing patient needs and generating treatment programs with established goals.
Position Responsibilities for a Home Health- PT:
* Evaluates patients to assess patient needs, conducts assessments necessary for creation of patients individual Plan of Care.
* Plans and implements individualized treatment program by identifying short and long term goals and methods to achieve identified goals.
* Supervises and directs licensed Physical Therapist Assistants in the implementation of individual patient’s POC.
* Reviews the quality and appropriateness of the visits delivered for effectiveness and efficiency through co-signing of PTAs visit notes.
* Provides treatment in accordance with established standards of practice.
* Consistently demonstrates sound judgement in evaluation, planning and implementation of patient therapy programs.
* Communicates in a positive, clear, professional manner with patients, families and other healthcare team members, especially when providing education.
Knowledge, Skills and abilities: Must meet all certification and licensure requirements as established by the state and national board associations. Must maintain an active license in good standing throughout employment.
