Summary:
The current Therapist vacancy is in the ReTurn Project which is part of Encompass Community Services Adult Integrated Behavioral Health service area. The ReTurn Project is an innovative collaboration between Encompass, Probation, and Sheriff’s Office of Santa Cruz County. ReTurn is known in the community and nationally for taking a client-centered treatment approach and supporting clients in successfully transitioning out of incarceration - improving their overall health, well-being, and community connection.

The ReTurn Project Therapist will provide mental health and co-occurring disorder treatment services to adults reentering after incarceration in collaboration with county probation. This position will be located at the Alto North Counseling Center office in Santa Cruz, however, travel is required throughout the county as clinicians meet with clients in the community or their homes. The Therapist must also be able to obtain jail clearance as the position requires the ability to provide services to clients who are currently incarcerated within all Santa Cruz County jail facilities. The Therapist will be responsible for maintaining a caseload of individual clients, facilitating evidence-based curriculum in both individual and group settings, completing intakes/assessments, maintaining client charts in a timely manner, ability to complete and report risk assessments, safety plans, and understand reporting procedures. This position is 24 hours per week and the program operates M-Fri between 8:30-5pm.
Duties and Responsibilities:
- Manage and maintain individual case load of clients.
- Provide individual sessions for mental health and co-occurring disorder clients
- Assists, collects, monitors, analyzes programmatic/client data
- Adheres to all reporting requirements utilizing current technologies
- Must communicate effectively and be able to use current technology in the following areas: case notes, assessments, treatment plans, incident reports, correspondence with community service providers
- Develop exit/transition plans with clients as treatment comes to closure
- Provide intakes and conduct assessments using the Adult Needs and Strengths Assessment (ANSA) and the Bio-Psycho-Social Assessment
- Have a working knowledge of the American Society of Addiction Medicine (ASAM) criteria
- Develop and maintain individualized treatment plans that are dictated by the ASAM/ANSA assessment
- Provide referrals for ancillary services and needs are assessed and completed
- Coordinate services with other health care providers as necessary
- Provide exit/discharge session
- Manage funding source authorizations and seek extensions when appropriate
- Act as a liaison with referral sources, i.e. county, probation
- Document client contacts, referrals, consultation with adjunct providers, probation officers, etc.
- Document treatment progress in paper charts or on an electronic health record.
Qualifications
- Masters level in Counseling or Social Work related field required
- Active Board of Behavioral Science registration or clinical license preferred
- A minimum of 1-2 years experience providing Mental Health treatment services to clients with co-occuring mental health and substance use issues and/or criminal justice system involvement.
- Experience with evidenced based practices for SUD and Mental Health treatment services.
- Experience working with clients in-custody or collaborating with corrections or probation agencies
- Working knowledge of DSM V and mental health diagnosis preferred
- Demonstrated work experience using a trauma informed approach in working with clients who have complex trauma, including substance use disorders, mental health disorders, and unstable housing
- Demonstrated work experience successfully working in collaborative teams
- Approaches challenges with curiosity, inquiry, and as a life-long learner
- Ability to complete documentation accurately and in a timely manner
- Strong skill set regarding clinical ethics, boundaries, and role clarity with clients
- Bilingual Spanish/English preferred
- Applicant must have active driver’s license and the ability to drive clients
- Applicant must submit appropriate background check clearance and be able to pass Santa Cruz County jail clearance to provide in-custody services as well as Probation Service Center background clearance.
- Experience with electronic health records preferred, strong computer literacy and writing skills
