Summary: 

Transition Age Youth Services serve current and former foster, probation placement and homeless youth ages 15-24. We provide an array of programs and services including counseling, skills building, a drop-in resource center, and subsidized supportive housing. Our mission is to support and empower youth in making a health transition into successful adulthood.  

The TAY ILP/Housing Manager reports to the Senior Program Manager and provides daily direction and supervision for a team of counselors and support staff who work in the TAY ILP/Housing Programs (THP Plus, THP FC, TVP), as well as occasionally providing clinical services and case management services to a small caseload youth and young adults. The TAY ILP/ Housing Manager will train and supervise TAY Coordinators in a safe, positive and supportive way that promotes their personal and professional growth and provide regular supervision and constructive feedback and direction as needed. The TAY team provides a wide spectrum of trauma informed services to empower and support youth and young adults to make positive choices and changes in their lives. The TAY ILP/ Housing Manager ensures the delivery of quality clinical and case management services in the housing programs as well as ensuring the clients are living in safe and habitable environments and their basic needs are provided for.

Responsibilities: 

  • Provides intervention services based on best practices & selected evidence based practices for mental health or substance use disorders and/or co-occurring disorders. 
  • Oversees ILP, TVP, THP Plus and THP FC Housing Programs – ensures programs are at full census, ensures program guidelines are being followed, ensures landlords are being checked in with on a regular basis to promote landlord retention and possible expansion of units. 
  • Ensures ILP, THP Plus and THP FC Data Tracking is completed each quarter.
  • Recruits, hires, and orients new staff to ensure job success, accurate understanding of job roles and responsibilities, and safety and compliance.
  • Provides regular supervision, constructive feedback and direction.
  • Must communicate effectively and be able to use current technology in the following areas: case notes, assessments, treatment plans, incident reports, correspondence with community service providers. 
  • Attends and represents TAY in a highly professional and ethical manner at community partner meetings; workgroups; events
  • Requests/reviews rental requests, allowance requests, and client assistance purchases. 
  • Manages Housing Application system 
  • Supports with data tracking and reporting, as needed.
  • Reviews and signs on behalf of TAY/Encompass with AFDC housing lease/agreements. 
  • Reviews housing participant applications and facilitates interviews for future participants. 
  • Supports housing lead with chart review 
  • Keeps track and updates monthly County Invoices for THP
  • Keeps track and updates monthly THP and NMD savings accounts for participants.
  • Supports Program Manager with maintaining Personnel Charts following CCL’s regulations. 
  • Supports Program Manager with CCL’s requirements 
  • Creates/organizes the On-Call Phone Calendar for TAY Housing programs and takes on-call shifts.
  • Collaborates with Clinical Supervisor with opening new clients to AVATAR. 
  • Receives and assigns ILP and housing referrals 
  • Checks for ILP and Housing eligibility with County Staff, Juvenile Probation or other States 
  • Participates in outreach meetings with community partners and represent TAY in a professional and ethical manner

Qualifications: 

  • 3-5 years of relevant experience working in social services or BA/BS in related field with 3 years of experience
  • Management/supervision experience required
  • Non-profit experience preferred
  • Bilingual (English/Spanish) preferred
  • AVATAR experience and MHRS certification preferred
  • Hours may vary – Typical shift consists of M-F 9:30-6:00 PM on-site schedule; may occasionally work in the field and need to start slightly earlier or stay slightly later to accommodate client and staff needs 
  • Ability to be a team player, leader, maintain effective working relationships and communicate in a respectful and professional manner when dealing with fellow employees, outside agencies and program participants/clients
  • Demonstrated record of showing initiative, solving problems, responding to constructive feedback, and ability to adapt positively to changing environments
  • Must have a vehicle, valid CA driver’s license, vehicle insurance, and good driving record to be covered by agency insurance and be willing to drive throughout Santa Cruz County
  • Fingerprinting for licensing, criminal record clearance and necessary physical exams are required
