Case managers will offer high-quality case management to clients enrolled in the L2H Program. The L2H program offers support to clients who have significant challenges such as experiencing homelessness, struggling with chronic health conditions, low income, former foster youth and substance use disorder. Case managers offer specialized support through our Enhanced Care Management and Community Support programs, focusing on health and housing.

- Preferred- experience and comfortability serving vulnerable populations
- Preferred- case management experience
Responsibilities:
- Hold a caseload of 24-28 clients
- Assessments, goal setting
- Creating and managing a care plan
- Referrals
- Supporting clients with health related goals
- Motivational Interviewing and De-escalation
- Care coordination including include warm hand-offs and closed-loop referrals
- Train in Evidence Based Practices by utilizing Encompass's client centered approach
- Documentation
- Client advocacy
Qualifications:
- BA/BS in Psychology or related field
OR
- HS/GED 2 or more years of experience with any of the following:
- working with individuals experiencing homelessness
- providing housing navigation
- case management services
- experience supporting individuals with co-occurring disorders
- Lived experience or knowledge of community resources greatly preferred
- computer skills, including navigating healthcare systems
- Must have valid CA driver’s license, clean driving record, and insured personal vehicle.
- Preferred: Masters in related field
- Preferred: Bilingual/bicultural
