Position Summary
Encompass’s Wraparound program is seeking a full-time Case Manager with experience working with adolescents. Wraparound is an innovative, research-based program that truly centers on youth voice and choice for a successful transition out of the juvenile justice system. The program has a tangible impact on the community in reducing recidivism and violence, while increasing resiliency and wellbeing. To reduce barriers, services are provided where most convenient for the client, which might be virtually, in families’ homes, at one of our offices, or in the community.
The Wraparound Case Manager is bilingual in English and Spanish and experienced working with system-involved youth and families. The role is responsible for supporting youth to improve their functional, social, communication, and daily living skills to enhance self-sufficiency and self-regulation. The Case Manager collaborates closely with County Probation Officers and as a dyad with the Transitional Specialist, attending Child and Family Team (CFT) meetings and billing some services to MediCal (experience with EPSDT documentation preferred). Bilingual in English/Spanish required, hourly pay differential for bilingual skills awarded upon qualification.
Minimum Qualifications & Requirements
A combination of experience, education, and/or training which demonstrates the following knowledge, skills, and abilities:
• Bachelor’s Degree in related area
• Experience working with systems-involved youth and families
• Bilingual English/Spanish required (verbal and written fluency in both languages)
• Experience with mental health services and MediCal billing preferred
• Communicate professionally and effectively verbally and in writing.
• Flexible schedule to meet clients’ needs. Some evening/weekend hours using rotating schedule for crisis support.
Essential Functions
• Direct Services: Implement Wraparound evidence-based model. Provide mental health rehabilitation services. Travel to community-based locations, including family homes, Encompass offices, Juvenile Hall, Probation offices (mileage reimbursed). Attend and facilitate Child and Family Team (CFT) meetings. Provide monthly coverage for the 24/7 crisis response phone line, which rotates across all staff. Additional compensation provided for on-call hours; overtime paid for hours worked while on-call.
• Documentation: Maintain audit-proof, quality documentation of services and data collection (i.e., psychosocial assessment, diagnosis, treatment plan, progress notes, etc.).
• Services Coordination: Work closely with the Wraparound treatment team, including participating in regular case conferences.
Schedule
The Wraparound Counselor position is full time with typical working hours (in order to minimize barriers, some evening and weekend hours might be required if a client/family is otherwise unable to meet).
