Wound Care Program Coordinator
- Completes wound assessment, develops appropriate plan of care, and initiates wound care regimen.
- Documents patient care problems, progress, and goals on the Plan of Care.
- Reviews and edits Plan of Care on a regular basis.
- Conducts wound rounds per hospital policy.
- Assesses and interprets patient's condition; reports abnormal findings to supervisor and physician.
- Seeks input from patients / families / referral sources on needs, quality, and satisfaction of care.
- Maintains a current list of patients with skin care needs.
- Orders appropriate specialty equipment for patients (e.g. bed, mattress) with supervisor approval.
- Identifies specific discharge needs for patients.
- Stays current via newspapers, periodicals, industry materials, associations, and the internet.
- Follows appropriate universal precautions during the delivery of care.
- Uses proper lifting and body mechanics while delivering care to patients.
- Administers medication per policy and licensure regulations and completes appropriate documentation.
- Identifies / stages / records wounds as required by the CMS Quality Reporting program.
- Knows detailed responsibilities required of position.
- Proactively learns new practices, services, regulations, programs, etc.
- Shares responsibility for gaps in product, practice, or regulatory knowledge.
- Understands and utilizes the company resource structure.
- Networks with hospital staff and peer-level staff to share best practices.
- Organizes, plans, and manages time effectively to complete assignments.
- Meets position requirements and performs essential functions.
- Successfully completes all assigned training and annual skills competency as determined by the hospital based on new responsibilities, specialized equipment, high risk / problem prone / or low volume procedures including but not limited to emergency response techniques.
