Wound Nurse (LPN/RN)

Position Summary:
Encore Wound Care is seeking a dedicated Wound Nurse (LPN/RN) to join our mobile wound care team. This role is designed to ensure that patients receiving advanced wound therapy maintain continuity in their plan of care throughout treatment. The Wound Nurse will travel weekly to nursing home facilities, working closely with nurse practitioners, facility staff, and our Wound Healing Solutions Department to optimize patient outcomes. Additionally, this role will include conducting in-service training for facility staff on wound care best practices, conservative management, advanced wound therapy, and preventative measures.
Key Responsibilities:
· Conduct weekly visits to nursing home facilities, focusing on patients receiving advanced wound therapy to monitor progress and adherence to the plan of care.
· Provide conservative wound care management and assist in wound dressing applications when needed.
· Collaborate with the Wound Healing Solutions Department to ensure timely and appropriate supply delivery for patients and facilities.
· Assist nurse practitioners during wound rounds, including bedside scribing and supporting documentation efforts.
· Perform chart reviews to ensure compliance with Encore Wound Care protocols and facility regulations.
· Assist in quarterly or as-needed skin sweeps at facilities to help identify potential wound risks.
· Conduct in-service training for facility staff on:
- Conservative wound care techniques
- Advanced wound care therapies and best practices
- Preventative measures for wound management, including pressure injury prevention strategies
- Supply management and proper dressing selection
· Educate facility staff on proper wound care management and act as a liaison between Encore Wound Care and nursing facilities to ensure the highest level of patient care.
Qualifications:
· Active LPN or RN license in the state of Ohio. Multi-state licensure a plus.
· Minimum 1 year of experience in wound care, long-term care, or skilled nursing preferred.
· Strong understanding of wound care principles, including advanced wound therapies, dressing selection, and conservative management techniques.
· Experience or willingness to provide staff education and training through in-services.
· Ability to work independently while collaborating with nurse practitioners, facility nurses, and administrative staff.
· Excellent organizational and communication skills with strong attention to detail in documentation.
· Comfortable traveling weekly to assigned nursing home facilities.
