What You'll Do :
As a Care Coordinator , you'll play a key role in ensuring quality care for the individuals we serve. Your responsibilities will include :
Client & Provider Support : Conduct home visits, match clients with providers, and monitor service quality
Case Management : Implement Person-Centered Service Plans (PCSPs) and advocate for client needs
Recruitment & Retention : Interview Direct Support Professionals (DSPs) and coordinate provider onboarding
Compliance & Documentation : Maintain records, update client files, and ensure compliance with state DDD & AHCCCS policies
Community Engagement : Educate families on HCBS services, maintain partnerships, and attend events
Who We're Looking For :
Experience as a Care Coordinator, HCBS Coordinator, Care Manager, Group Home Manager, or Direct Support Professional
Passion for serving individuals with developmental disabilities
Strong case management, advocacy, and collaboration skills
Knowledge of Arizona's DDD programs and service systems
Proficiency in Google Drive & Apple products
Valid Level One fingerprint clearance card
Ability to travel locally (must have a valid driver's license & personal transportation - mileage reimbursed)
Must pass background checks, including CPS & APS registries
