We are looking for acute care pediatric trained nurse. We are looking for on call coverage for our pediatric hospice clients. Minimum of 2 years of acute pediatric care training is required.
JOB SUMMARY:
Pediatric Nurse Case Manager (RNCM) provides direct nursing care to the Endless Journey, LLC life-limiting pediatric and neonatal clients in all settings. This position provides support and understanding to the family throughout the client’s time enrolled in hospice services and in time of bereavement. The individual works as a member of the hospice team in providing end-of-life care to children with life-limiting conditions and their families. This position includes a balanced weekday on call, rotating weekend on call, and supplementary holiday on call. The Pediatric RNCM reports to the Director of Pediatrics.

Supervised by: Director of Pediatrics
Positions Supervised: Hospice Aides
Interrelationships: Clients, families, Pediatric Care team, IDG and hospice team members
QUALIFICATIONS:
Educational/Degree: Graduate from an accredited school of nursing
Training/Licensure: Currently licensed as a Registered Nurse to practice in the state.
Maintain/Complete training and certifications specific to position
Knowledge/Skills/Ability: Ability to work independently, make accurate, and at times, quick judgments. Ability to supervise others appropriately. Ability to respond appropriately to crisis outside of a hospital setting. Acceptance of and adaptability to different social, racial, cultural, and religious modes. Completes Hospice training program.
Experience: Minimum 2 years of experience as a Registered Nurse working in pediatrics. Preference for experience in acute care pediatric settings (ie PICU, CICU, NICU, Emergency Department)

Physical Requirements:
Requires considerable physical effort most of the day including kneeling, squatting, reaching, twisting, climbing, walking, exposure to temperature and humidity changes and maximal assist in lifting and/or transferring. Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position. Employee must provide evidence of annual TB test and other state-required tests or examinations.

Transportation:
Must have a current valid driver’s license, auto liability insurance and reliable transportation.
Essential Functions:
- Regularly assesses and reassesses the nursing needs of the pediatric hospice client.
- Provides hospice nursing services, treatments, and preventive procedures.
- Initiates nursing procedures appropriate for the client’s hospice care and safety.
- Develops and re-evaluates the client/family care plan in conjunction with IDG to meet needs and maintain continuity of care.
- Observes signs and symptoms and reports to the appropriate peds team disciplines any unexpected changes in the patient’s physical or emotional condition.
- Provides education, supervises, and counsels the client and family members about providing care and disease process.
- Supervises and trains other nursing service personnel within pediatric scope of practice.
- Develops and re-evaluates the client/family care plan in conjunction with IDG to meet needs and maintain continuity of care.
- Attends Pediatric Care team meetings as assigned and participates in interdisciplinary coordination of care
- Maintains records as required by Endless Journey policy.
- Follows the policies and procedures of Endless Journey. Observes confidentiality and safeguards all patient-related information in compliance with HIPAA regulations.
- Communicates to the supervisor if unable to meet a patient’s need or perform a procedure.
- Provides dietary counseling as it relates to changes that occur during EOL.
- Participates in on-call schedule and is responsible for providing on-call coverage when assigned
- Maintain skills and knowledge.
- Evaluation and admission of clients.
- Organizes work schedule and utilizes time management to be able to attend all required meetings.
- Assist with orientation, teaching and training as requested.
