The patient assessment coordinator (PAC) is responsible for the data collection for the Inpatient Rehab. Facility Patient Assessment Instrument (IRF-PAI) on the inpatient rehab unit. This position ensures the presence of supporting documentation to accurately complete the IRF-PAI to meet CMS standards. The PAC provides staff education in required documentation standards for the IRF-PAI. Coordinates with physician providers to ensure accurate coding of rehab impairment codes and co-morbidities.
Essential Duties and Responsibilities:
- Collects data for completion of the IRF-PAI.
- Oversees all aspects of the IRF-PAI and ensures accuracy of data.
- Confers with the clinical team for resolution of any inconsistences in the documentation.
- Assists the PPS Manager as a resource for problem solving in regards to Medicare’s prospective payment system (PPS) for the inpatient rehab units.
- Provides education to the clinical team on all changes in the PPS and documentation requirements.
- Submits IRF-PAI data in a timely manner to CMS.
- Reviews documentation with the medical staff to ensure that all co-morbidities are accurately documented in the medical record.
- Responsible for submission of quality reporting program data to CMS.
- Follows organizational and hospital policies and procedures.
- Performs other duties as requested.

Salary : $43,000 - $45,000
