We are currently seeking a for Service Nurse to join our team who will provide home care services in the Manhattan and Bronx areas!
The Qualified Candidate Will
- Provide in-home care management for complex chronically ill clients
- Provide a full range of nursing care duties in the client’s home including assessment, education and medication management
- Coordinate client’s care with care team and create care plan for client’s care
- Document care in EMR in a timely manner
- A valid NYS RN license
- A minimum of one (1) year home care experience as an RN with a CHHA or LHCSA
- A minimum of one (1) year in an acute care or hospital environment
Salary : $50
