Job Summary :
The Case Coordinator is responsible for incorporating several professional functions into the management of the total episode of care for patients on their assigned unit(s). She / he uses independent judgment in connection with her / his managerial responsibilities.

Status : Full-time
Shift : 8 hour days
Hours : 9a - 5p
Job Essentials
- Requires a minimum of 3 years of case mangagement experience in a hospital setting and / or home health care nursing experience
- Excellent documentation and communication skills.
- Ability to work flexibly.
- Proficiency in computer skills.
