Responsibilities:
As a Care Coordinator - Community Health Specialist, you will provide coordination of care, ensure continuous assessment of each patient's and family's needs, and implement the interdisciplinary care plan.
You will perform services in accordance with the physicians orders and the established plan of care (POC), ensuring delivery of care is compliant with federal and state regulations.
Qualifications:
You must be a graduate of an approved school of clinical education, licensed in the state where you currently practice, and have at least one year experience as a licensed professional.
You must also have basic demonstrated technology skills, including operation of a mobile device, and possess a valid state driver license, maintain automobile liability insurance, and maintain dependable transportation in good working condition.
