Role Description
This is a full-time on-site role for a Care Transitions Coordinator, located in Oklahoma City, OK. The Care Transitions Coordinator will be responsible for coordinating patient care transitions, collaborating with healthcare providers, managing case files, and ensuring that patient needs are met during the transition process. They will conduct assessments, develop care plans, and communicate effectively with patients, families, and the healthcare team.
Qualifications
- Strong Communication skills and the ability to effectively communicate with patients, families, and healthcare providers
- Case Management experience, including coordinating care and managing patient transitions
- Knowledge of Home Care practices and procedures
- Experience in Training and educating patients and families
- Sales skills, including the ability to promote the benefits of home health and hospice services
- Excellent organizational and time-management skills
- Bachelor's degree in Nursing, Social Work, or a related field
- Experience in the healthcare industry, particularly in home health or hospice care, is preferred
