We are seeking a Clinical Team Leader for our Home Health & Hospice team. The ideal candidate will be responsible for coordinating home health services, ensuring compliance with standards of practice, and providing oversight of patient care. This role may also involve participating in an on-call rotation and making patient visits.
Qualifications
- Graduate of an accredited school of professional nursing
- Licensed in the state of employment
- Minimum of two years nursing experience and one-year experience in home health
- Good understanding of federal, state, and local laws / regulations for Home Health
- Skilled with technology and EMR platforms
- Valid state driver's license and automobile liability insurance
