Job Description
We are seeking an experienced Registered Nurse to join our team as a Home Health Care Coordinator. In this role, you will ensure the overall coordination of home health services provided to patients, delivering care according to acceptable standards of practice and company procedures.
You will review and approve patient information submitted by licensed professionals during start of care, recertification, resumption of care, or evaluation visits. Your primary goal will be to facilitate relationships between physicians, referral sources, patients, caregivers, and employees.
