About the Role
The Registered Nurse (RN) Case Manager is the point of contact for all disciplines involved with providing care to patients and oversees the frequency of visits for the episode. This role requires performing accurate OASIS collection, ensuring the medication profile remains current, and reporting lab values to the physician timely.
Key responsibilities include attending weekly case conference and monthly case manager meetings, ensuring seamless communication among team members. The ideal candidate will possess excellent interpersonal and problem-solving skills, with the ability to navigate complex situations effectively.
