The Home Health Weekend Registered Nurse will be responsible for:
- Performing skilled nursing visits and coordinating client care,
- Serving as the point of contact for all disciplines involved in patient care,
- Overseeing visit frequency for each episode,
- Consulting with physicians and offices regarding patient care,
- Collecting accurate OASIS data and ensuring medication profiles are up-to-date,
- Reporting lab values to physicians in a timely manner,
- Attending weekly case conferences and monthly case manager meetings.
To be considered for this role, you must:
- Be currently licensed as a Registered Nurse (RN) in the state of employment,
- Have a minimum of one year of clinical experience preferred,
- Demonstrate knowledge and skill in current nursing practice,
- Possess a valid state driver's license and automobile liability insurance.
