As a Registered Nurse (RN), you will be the primary point of contact for client care, overseeing the frequency of visits and coordinating with other disciplines involved in patient care. You will also consult with physicians and office staff to provide accurate updates on patient care.
Additional responsibilities include:
- Accurate OASIS collection
- Current medication profile management
- Timely reporting of lab values to physicians
- Regular attendance at weekly case conferences and monthly case manager meetings
To be successful in this role, you must meet the following requirements:
- Be currently licensed as a Registered Nurse (RN) in the state of employment
- A minimum of one year of clinical experience is preferred
- Demonstrate knowledge and skill in current nursing practice
- Possess a valid state driver's license and automobile liability insurance
