Responsibilities
In this role, you will provide essential patient care, oversee client coordination, and ensure high-quality care delivery. Key responsibilities include:
- Performing visits and completing coordination of client care
- Serving as the point of contact for all disciplines involved with providing care to patients
- Overseeing the frequency of visits for the episode
- Consulting with physicians and office staff regarding patient care
Qualifications
To be successful in this role, you must meet the following requirements:
- Be currently licensed as a Registered Nurse (RN) in the state of employment
- Have a minimum of one year of clinical experience preferred
- Demonstrate knowledge and skill in current nursing practice
- Possess a valid state driver's license and automobile liability insurance
