As a RN Case Manager, you will be responsible for coordinating patient care, performing visits, and collaborating with physicians and office staff to ensure accurate and timely care. You will also be responsible for collecting accurate OASIS data, maintaining up-to-date medication profiles, and reporting lab values to physicians.
