Job Description
As a Registered Nurse (RN) Case Manager, you will be responsible for performing visits, completing coordination of client care, and serving as the point of contact for all disciplines involved in providing patient care.
You will consult with physicians and office staff as needed, perform accurate OASIS collection, ensure medication profiles remain current, and report lab values to physicians in a timely manner.
We require a minimum of one year of clinical experience, valid state driver's licenses, and automobile liability insurance.
