What You’ll Do:
- Transports consumers within the community.
- Supports consumers in achieving personal care goals and addressing ongoing challenges.
- Adheres to the Adult Mental Health Rehabilitation Services standards (AMHRS).
- Respond promptly to consumer concerns, ensuring high satisfaction.
- Educate consumers on medications, track logs, and conduct monthly audits.
- Coordinate services based on individual needs.
- Deliver high-quality care in line with Individual Rehabilitation Plans (IRP).
- Accurately document services within required timeframes using electronic health records.
Job Requirements:
- Bachelor’s Degree in Mental Health, or Human Services OR
- A combination of one (1) or more years of college, plus related life or work experience equal to four (4) years OR
- High School or GED plus four (4) years of related work or life experience.
- Intermediate computer skills with the ability to learn new computer programs.
- Valid Driver’s License and an acceptable driving record.
- Strong, effective verbal and written communication skills.
- A compassionate, flexible, and client-focused approach with all consumers.
