Position Summary
The Hospice RN is responsible for leading, coordinating, and delivering comprehensive care aligned with the patient’s plan of care. You will serve as a key member of our care team, supporting patients and families physically, emotionally, and spiritually throughout their journey.
Key Responsibilities
Conduct initial and ongoing assessments for hospice patients
- Develop and maintain individualized care plans
- Collaborate with physicians, hospice aides, and interdisciplinary team members (IDG)
- Provide direct patient care and educate family members on care and the dying process
- Monitor and evaluate patient responses to care plans and adjust as needed
- Maintain accurate, timely, and compliant clinical documentation
- Participate in IDG case conferences, QAPI meetings, and staff development sessions
- Perform on-call duties, including nights, weekends, and holidays
- Supervise Hospice Aides and LPNs, including monthly evaluations
- Provide bedside care when hospice aide is unavailable
Graduate of an accredited School of Nursing
- Active Registered Nurse License in the State of Virginia
- Minimum 1 year of patient care experience ( preferred) / New Grads welcome to apply.
- Valid Driver’s License and up-to-date Auto Insurance
Strong communication, documentation, and problem-solving skills - Knowledge of hospice regulations (Medicare/Medicaid) and end-of-life care standards
- Ability to lift and move up to 250 lbs safely (with assistance) and travel to patient homes
