Qualifications (required at time of offer) :
- Bachelor's degree in social work, human services, psychology, or related field required.
- Previous experience in case management, social services, or community outreach preferred.
- Strong interpersonal and communication skills, with the ability to engage and build relationships with diverse populations.
- Knowledge of community resources, social services, and support programs.
- Ability to work independently, prioritize tasks, and manage time effectively.
- Compassionate, empathetic, and non-judgmental approach to client interactions.
- Proficiency in Microsoft Office suite and familiarity with database management systems.
- Valid driver's license and reliable transportation required.
General Description
The Case Manager at Families and Children Together (FACT) plays a pivotal role in supporting vulnerable families by coordinating and facilitating access to community resources and services. This position serves as a primary point of contact for families referred to FACT, working collaboratively with internal teams and external stakeholders to ensure comprehensive support and assistance.
