Company Overview

At Ennoble Care, we are committed to providing exceptional dementia care and caregiver support. Our Memory Care Program focuses on empowering care partners with the tools, education, and resources necessary to navigate dementia care effectively. We are currently seeking a full-time Memory Care Coordinator to join our team on a hybrid schedule, based out of our Voorhees, NJ office. This role provides remote care coordination and support services, with some in-person collaboration as needed. We welcome candidates from diverse backgrounds and offer training to equip them with the skills necessary for success.


Job Summary

The Memory Care Coordinator is responsible for care coordination, caregiver support, and dementia education, ensuring that patients and their care partners receive the guidance they need. This position involves remote engagement with care partners and healthcare teams, supporting them through education, resource navigation, and ongoing case management. The ideal candidate will have hands-on experience in dementia care, with a deep understanding of the challenges faced by both patients and their care partners. Effective communication, empathy, and the ability to offer emotional support are essential to success in this role While prior experience in dementia care is valuable, we are willing to train the right candidate with strong communication skills, empathy, and a passion for caregiving support.


Responsibilities

Care Coordination & Care Partner Support

  • Act as a remote point of contact for care partners, guiding them through dementia-related challenges and connecting them to appropriate resources.
  • Develop individualized care plans, conduct assessments, and collaborate with healthcare providers to ensure comprehensive, person-centered support.
  • Provide education and emotional support to care partners, equipping them with strategies to manage dementia-related behaviors and navigate care decisions.
  • Identify and recommend community resources such as respite care, support groups, and memory care programs.

Virtual Case Management & Communication

  • Maintain regular phone, video, and digital communication with care partners, ensuring ongoing support and intervention as needed.
  • Maintain open communication with the Memory Care Program Manager, regularly meeting to discuss patient care, care partner goals, and interventions. Provide updates on participant progress, engage in case discussions, and coordinate necessary adjustments to care plans to ensure efficient support and alignment with the care process. Foster collaborative communication with internal and external teams to meet patient and care partner needs.
  • Document all interactions, assessments, and recommendations within electronic medical record (EMR) systems for accurate record-keeping and compliance.

Care Partner Advocacy & Resource Navigation

  • Provide empathetic, responsive, and professional service in all interactions with patients, care partners, and colleagues. Demonstrate compassion and empathy in all communication, ensuring that patients and their care partners receive the highest level of support.
  • Serve as an advocate for care partners, helping them access services, understand dementia progression, and make informed decisions.
  • Offer virtual educational sessions and caregiver coaching, supporting families in their caregiving journey.
  • Coordinate with internal and external teams to ensure care partners receive seamless, efficient support.

Technology Utilization & Documentation

  • Utilize telehealth platforms, secure messaging systems, and internal databases to streamline communication and case management.
  • Maintain accurate, timely documentation in compliance with company policies and regulatory requirements.

Team Collaboration & Professional Development

  • Work closely with healthcare providers, memory care specialists, and program leadership to refine care coordination strategies.
  • Actively participate in virtual team meetings and in-person training sessions as required.
  • Contribute to a collaborative, mission-driven environment, supporting team goals and enhancing the Memory Care Program's impact.

Qualifications

Experience & Knowledge

  • Experience working with individuals diagnosed with dementia or other memory-related conditions, preferably in a senior or geriatric care setting. Personal experience as a care partner for individuals with dementia is also highly valued. A foundational understanding of dementia and memory-related conditions is preferred.
  • Familiarity with basic healthcare terminology, conditions, and care principles to effectively understand patient needs and care plans.
  • Knowledge of local resources for dementia patients and care partners, such as respite care, support groups, and medical specialists.

Communication & Technology

  • Comfortable with frequent phone, video, and digital communication to engage care partners and healthcare teams.
  • Strong verbal and written communication skills to provide clear, compassionate guidance.
  • Proficiency in electronic medical records (EMR), email, and virtual communication platforms is beneficial—training available.

  • Emotional Resilience: Must be emotionally resilient to manage the emotional demands of working with patients with dementia and their families, while maintaining personal well-being.
  • Professionalism & Team Collaboration: Maintains a positive attitude, adapts to change, and collaborates effectively with others to support team goals and patient care outcomes.
  • Goal-Oriented & Self-Motivated: Driven to meet individual and team goals, proactively working to improve performance while maintaining high standards of care. Demonstrates a dedication to continuous learning and development in areas such as dementia care, caregiver support, and healthcare coordination.

Work Environment & Flexibility

  • Hybrid schedule: This role allows for a combination of remote work and in-person collaboration at our Voorhees, NJ office.
  • Must be comfortable and compliant in working independently while maintaining high engagement with a virtual care team.
  • Some flexibility in schedule is preferred to accommodate care partners in different time zones.
