We are currently seeking a compassionate and skilled full-time Memory Care Coordinator that will work out of our Voorhees, NJ office on a hybrid schedule to join our team and help us achieve our mission.
Job Summary :
The Memory Care Coordinator is responsible for coordinating, managing, and supporting the care of individuals living with dementia. This role focuses on ensuring that patients' healthcare needs are met, while also providing emotional and educational support to their care partners. The ideal candidate will have hands-on experience in dementia care, with a deep understanding of the challenges faced by both patients and their care partners. Effective communication, empathy, and the ability to offer emotional support are essential to success in this role.
Responsibilities :
- Care Coordination & Memory Care Participant Support : Develop individualized care plans, conduct assessments, and collaborate with healthcare providers and internal teams to ensure comprehensive care for program participants. Provide education and emotional support to patients and their care partners, offering strategies for managing dementia-related challenges, and ensuring they feel empowered and equipped to care for their loved ones. Identify appropriate resources, such as respite care and support groups, to support both patient and care partner well-being.
- Communication & Case Reporting : Maintain open communication with the Memory Care Program Manager, regularly meeting to discuss patient care, care partner goals, and interventions. Provide updates on participant progress, engage in case discussions, and coordinate necessary adjustments to care plans to ensure efficient support and alignment with the care process. Foster collaborative communication with internal and external teams to meet patient and care partner needs.
- Customer Service & Emotional Support : Provide empathetic, responsive, and professional service in all interactions with patients, care partners, and colleagues. Demonstrate compassion and empathy in all communication, ensuring that patients and their care partners receive the highest level of support.
- Problem-Solving & Care Coordination : Utilize internal systems and tools to address and resolve patient care concerns effectively, ensuring smooth care delivery. Apply critical thinking skills to adapt care plans and address evolving patient and care partner needs, ensuring seamless care coordination and service delivery.
- Team Collaboration & Support : Actively contribute to team success by offering support, collaborating to meet departmental goals, and working with both internal and external teams to foster a collaborative approach to patient care. Contribute to creating a positive, team-oriented work environment that ensures high-quality care.
- Technology Utilization & Documentation : Become proficient in using multiple platforms and systems, including secure email, telephone systems, electronic medical records (EMR), and internal knowledge base materials. Maintain accurate, timely documentation in compliance with company protocols and regulatory standards, ensuring effective care coordination and communication.
