Join a dynamic Supported Living agency and work with a team of Case Managers that are supportive, hardworking and love their work.
The Case Manager is responsible for the coordination of supported living services for adults with developmental disabilities as assigned, with a maximum caseload of 10 individuals. Schedules for support staff, live-in staff and IHSS staff are coordinated and assigned by the Case Manager.
The Case Manager, in coordination with the Director and Associate Director, will supervise, train, monitor and evaluate all paid support staff. It is critical that the Case Manager understand, model, maintain and be able to articulate the philosophies, values and goals of supported living as it relates to each individual and his/her lifestyle.
The Case Manager will be part of a team that includes the individual receiving services, service professionals, family members and others identified in the Individual Support Plan (ISP).
Attend quarterly meetings with SD Regional Center, client and circle of support. Help remedy issues with MediCal, CalFresh and other programs. Review time off requests for staff and arrange for coverage. Use a scheduling software program to input schedules and review timesheets each pay period for accuracy.
Visit with clients and staff on a regular basis. Respond to texts and emails in a timely manner, may work occasional shifts.
Excellent written and verbal communication skills are a must!
Job Type: Full-time 3 days in office (10am-3:30pm), 2 days work from home
Salary: $28.00-$31.00/hour DOE mileage & phone reimbursement
