Job Description:
We are seeking a dedicated and detail-oriented Intake Coordinator to join our dynamic team. The ideal candidate will have a solid background in home health, hospice care, or patient intake processes. This role requires proficiency in electronic medical records (EMR) systems, specifically Kinser and Axxess and Dr. kronos.
Key Responsibilities:
- Manage the initial screening and intake process for new patients.
- Coordinate with healthcare professionals to ensure seamless patient admissions.
- Maintain accurate and up-to-date patient records within EMR systems.
- Provide exceptional customer service to patients and their families.
- Ensure compliance with all federal, state, and local regulations.
Qualifications:
- 3 to 5 years of experience in home health, hospice, or patient intake.
- Proficiency in EMR systems, particularly Kinser and Axxess.
- Strong organizational and communication skills.
- Ability to work independently and as part of a team.
- Bilingual abilities are preferred but not necessary.
