About the role
Entrust Community Services is seeking a compassionate and dedicated Case Manager to provide comprehensive case management services to individuals and families across all programs. This role involves developing individualized service plans, fostering supportive relationships, and advocating for participants with external agencies. What you'll do
  • Service Delivery & Planning:
    • Partner with Intake Outreach Case Aid for assessments.
    • Develop and implement individual and family service plans.
    • Perform periodic reassessments and update service plans.
  • Participant Advocacy & Support:
    • Maintain positive, supportive relationships with participants and community partners.
    • Advocate for participants with agencies and resource providers.
    • Ensure participants’ needs are met by identifying and securing necessary resources.
  • Documentation & Communication:
    • Document participant progress and plan updates.
    • Communicate effectively with internal teams and external partners.
    • Prepare and distribute reports and ensure team members understand their roles in the service plan.
  • Collaboration & Coordination:
    • Work closely with internal staff and external entities to ensure compliance with program rules.
    • Coordinate coverage with team members in case of absences.
    • Assist in exiting participants from programs according to procedures.
  • Research & Resource Development:
    • Identify and research community resources to support participants.
    • Stay informed on advocacy issues, human services systems, and external resources.
Qualifications
  • Education & Experience:
    • Bachelor's degree in psychology, Sociology, Special Education, or related field with 1 year of experience.
    • OR Associate degree in a related field with 2 years of experience.
    • OR High school diploma/GED with 3 years of case management experience.
  • Skills & Abilities:
    • Strong knowledge of service plans and community resources.
    • Ability to develop and implement individualized plans and complex reports.
    • Proficient in documentation, assessment, and advocacy.
    • Excellent verbal and written communication skills.
    • Ability to interpret technical reports and solve practical problems.
  • Technical Proficiency:
    • Familiarity with Microsoft Office Suite (Excel, Outlook, Word, PowerPoint) and database software (SET-Works).

REQUIREMENTS:

  • Current First Aid/CPR certification (preferred).
  • Valid Washington State driver’s license and proof of automobile insurance.
  • Ability to pass a Washington State Criminal Background Check, drug screening, and E-Verify.

WORK ENVIRONMENT:

  • Office setting with occasional travel required.
  • Typical schedule: Monday to Friday, 8:00 AM - 4:30 PM, with a 30-minute lunch. Hours may vary.
