Case Manager – Structure Family Caregiving (SFC)
What You Will Do:
- Conduct comprehensive initial home assessments, ensuring all necessary caregiver credentials are in place and required home modifications are complete.
- Day to day monitoring relating to patient enrollment, credentialing, claims operations, prior authorization appeals, program integrity and reporting related to the SFC programs and the ability to triage issues to the appropriate LTSS internal staff.
- Work with members/family to resolve issues that relate to the SFC provider network and respond to requests for information regarding payment and policy questions.
- Gather essential information, perform psycho-social evaluations, and facilitate referrals to appropriate healthcare and community services.
- Coordinate patient placement in compliance with established procedures.
- Collaborate with caregivers and the care team to define and implement individualized care plans.
- Perform regular home visits to supervise personal care services and review care plan progress.
- Maintain detailed and accurate case records, documenting significant changes and observations.
- Provide ongoing training and support to caregivers.
- Collaborate effectively with nurses, primary care physicians, and other medical providers to ensure timely and appropriate delivery of consumer services.
- Actively participate in discharge and transition planning.
- Conduct annual reviews to ensure the continued suitability of the qualified setting.
- Maintain continuous survey readiness and ensure compliance with all regulatory and accreditation standards.

What You Will Bring (Required)
- Bachelor's degree required, must be LSW or LISW, human services, social work, or a related field.
- At least one year of experience working with elders or adults with disabilities.
- Valid driver's license and automobile insurance, as travel within Ohio is required.
- Demonstrated experience in case management, care planning, and assessment in a healthcare setting.
- Outstanding proficiency in common software and web-based applications including Microsoft Excel, Word, PowerPoint,and Teams.
- Master's degree in social work or a related field.
- Experience working in Structure Family Caregiving (SFC) or a similar setting.
- Certifications in gerontology or case management.
- Familiar with using Hubspot.
