We are seeking an experienced and compassionate Full-Time, Licensed Clinical Social Worker (LSWAIC or LICSW) to provide services to hospice patients in Kitsap County, WA, and surrounding areas, including homes, assisted living facilities, and skilled nursing facilities.

Essential Duties:
- The Social Worker in this role will act as an advocate and help patients foster their capacities and coping mechanisms to improve and better their daily lives and experiences.
- Assess the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment.
- Participates in the development of the individualized care plan, involving the patient and family/caregivers, and attends regularly scheduled interdisciplinary group meetings.
- Assist the IDG team in recognizing the effects of psychosocial stresses and symptoms of the terminal illness.
- Communicate all clinical findings to the registered nurse and other members of the interdisciplinary group.
- Complete an assessment of the patient's identified residence and provide direction and suggestions if identified as unsafe or if another plan is required.
- Carries out social evaluations, including family dynamics, cultural requirements, caregiver abilities, communication patterns, suicide risk, neglect or abuse, and intervention plans based on evaluation findings.
- Guides patients and family/caregivers as needed in relationship to their stress, and other identified coping opportunities.
- Provides crisis intervention when necessary.
- Provides information and referral services for organization patients and families/caregivers regarding practical and environmental needs and addresses advanced directives.
Qualifications:
- A graduate of a master's program in social work accredited by the Council on Social Work Education.
- Must be a Licensed Clinical Social Work (LICSW or LSWAIC)
- Must have at least one (1) year of experience in health care and exposure to patients facing hospice is preferred.
- Must have experience with grief counseling or bereavement.
- Understand hospice philosophy.
- Demonstrates good verbal and written communication and organization skills.
- Current CPR Certification.
- Must be an insured licensed driver with an automobile that meets organization requirements and is in good working order.
